As we mark the third anniversary of the Russian invasion of Ukraine, the repercussions of the conflict extend significantly beyond the battlefield. The war has resulted in substantial civilian casualties, with the United Nations reporting 40,838 casualties, including 12,456 fatalities, and has displaced millions, leading to an urgent humanitarian crisis affecting approximately 17.6 million individuals in Ukraine.

The Ukrainian healthcare system, previously a symbol of resilience, has been severely strained, facing unprecedented challenges. The World Health Organization has documented over 2,215 attacks on healthcare facilities, resulting in 205 deaths and 699 injuries. These attacks, coupled with security concerns and shortages of medical supplies and personnel, have hindered access to essential medical care, particularly for stroke prevention and treatment. Notably, despite a reduced population, acute stroke admissions in 2023 increased by 13% compared to 2021, driven by poorly managed risk factors and the impacts of displacement.

In response to these challenges, the European Stroke Organisation (ESO) established a Task Force for Ukraine (TF4UKR) in March 2022, aimed at supporting stroke patients and healthcare professionals. The TF4UKR has implemented various initiatives, including waiving membership fees, facilitating the movement of Ukrainian stroke physicians to European institutions, offering grants for professionals to engage with stroke units across Europe, and organizing a stroke conference in Liviv in September 2024.

Collaborative efforts among professional stroke societies, the Ukrainian Ministry of Health, and international organisations have led to significant advancements in stroke care during the conflict. National medical standards have been developed, and improvements in the rates of intravenous thrombolysis and mechanical thrombectomy have been observed: thrombolysis and mechanical thrombectomy in ischemic stroke have risen, last year exceeding 10% and 2%, respectively.

Furthermore, with support from the World Bank, Ukraine secured a substantial loan to invest in state-of-the-art medical equipment, including approximately 200 CT scanners, dozens of MRI machines and angiographic suites. These proactive measures laid a strong establishment for stroke treatment, ensuring that essential infrastructure and pathways were in place. Despite the challenges that followed, these initiatives continued to evolve, driving meaningful improvements in stroke care delivery across the country.

The ESO’s ongoing commitment to supporting Ukrainian stroke patients exemplifies a broader European solidarity, highlighting the need for continued attention to the healthcare challenges faced by vulnerable populations amid the ongoing crisis. As we reflect on this anniversary, it is crucial to recognise the profound human cost of the war, particularly for those affected by strokes, while also acknowledging the hope and resilience fostered by international collaboration and support.

This month marks the second anniversary of the war in Ukraine. This is a devastating crisis for Ukraine and for Europe. Large part of the population fled the country seeking refuge in neighboring and far countries, mostly women and children. Leaving the most intense conflict zones, survivors were pushed into degraded living conditions, which presented a unique strain upon the healthcare system, that found no parallel in recent history of Europe.

According to the United Nations Office for Human Rights, 27,768 civilian casualties (9,806 fatalities and 17,962 injured) were reported between February 24, 2022, and October 8 2023. An estimated 17.6 million people in Ukraine require urgent humanitarian support, over 6 million Ukrainian residents became internally displaced persons, and 6.3 million refugees from Ukraine were recorded globally, most of whom (5.9 million) reside in Europe.

With respect to stroke care, the health and wellbeing of the Ukrainian population was heavily affected. Access to thrombolysis and thrombectomy, stroke units, and to services for secondary prevention rehabilitation and palliative care has been affected in conflict regions, especially for vulnerable patients such as stroke survivors and their families.  Before the conflict Ukraine was among the top five countries in Europe for stroke incidence, morbidity, and mortality: each year, there are 100–110,000 primary and 40–50,000 recurrent cases of stroke. Because of this high burden, stroke remains a priority for the Ukrainian Ministry of Health and National Health System. Investments in new equipment allowed all Ukrainian regions to offer mechanical thrombectomy (MT) to acute ischemic stroke patients. The MT rate is 1.5%, whereas access to thrombolysis is 10%. In 2024 new policies were issued to ensure improved access and quality of inpatient rehabilitation, increasing the number of days (up to 112) covered by the NHS.

In 2022, the European Stroke Organisation (ESO) established the Task Force for Ukraine (TF4UKR) chaired by Dr. Francesca Romana Pezzella (Italy) and Dr. Yuriy Flomin (Ukraine) to provide a dedicated team providing a continuous channel for the Ukrainian stroke community to network effectively with European national Societies, healthcare professionals and the WHO.

The ESO TF4UKR organized educational and communication events and funded grants in favor of Ukrainian stroke professionals. Among the main activities conducted in the last 2 years are: 1) supporting Ukrainian stroke physicians to relocate to other European institutions; 2) offering grants to Ukrainian Neurologists to visit Stroke Units in other European countries in 2023; 3) building educational forums, including virtual educational event series; 4) forming collaborations with WHO and other European Institutions, such as the University of Oslo, the Danish Stroke Society and Stroke Support Organization, to address gaps in stroke care, using mapping services and providing equipment. ESO’s solidarity with the Ukrainian stroke community remains steadfast. We trust that this collaboration will reinforce the link between our communities and provide opportunities for growth and recovery of Ukrainian stroke healthcare systems.

Building Bridges

The European Stroke Organisation deplores the war in Ukraine and stands by its position that the Russian invasion in Ukraine and indiscriminate attacks by Russian military forces are unjustified and wrong.

Since the very first days of the conflict, ESO has been supporting actively stroke victims and stroke healthcare workers in response to the conflict in Ukraine by with multiple initiatives. Among these, ESO has established a dedicated task force, which has carried out successfully a series of significative actions under different domains:

Communication and Coordinated Support:

  • Creation of Telegram groups for direct communication among members of Ukrainian stroke organisations and ESO members to empower immediate response
  • Coordination with WHO for joint support
  • Circulation of surveys to assess and address urgent medical needs – the results have also been used to write a paper (under evaluation) to describe Ukrainian stroke system response during the first term of the conflict.

Financial Initiatives:

  • free ESO membership for Ukrainian colleagues
  • free access to virtual ESOC 2022 for Ukrainian colleagues connecting from Ukraine
  • 15 grants to support participation to ESOC 2023 in Munich
  • 10 grants to Ukrainian Neurologists to visit Stroke Unit in a European country- read their stories on our website pages
  • Rehabilitation materials delivered to Ukraine – we are very thankful to Professor Hanne Christensen (Copenhagen, Denmark) for this successful operation!

Patients support:

  • Formal and informal networking to support stroke victims in needs of rehabilitation, drugs medical support in Ukraine and abroad – we are very thankful to Professor Giancarlo Graziani, San Filippo Neri Hospital, Rome, for having received the first Ukrainian stroke patient in his rehabilitation unit in March 2022.

Cultural initiatives: medical education and research:

  • Two Webinar series for Ukrainian colleagues addressing the specific needs of Ukrainian doctors with simultaneous Ukrainian translation, a third series focusing on clinical cases and challenging stroke research issues – we are very thankful to Angels for offering the technical support and simultaneous translation, and to Professor Charlotte Sandset  (Oslo, Norway) for developing, organizing and keep working this project aided by Dr. Pavlo Lebedynets ( Kiev, Ukraine).
  • University of Oslo Awards Grant to Task Force 4 Ukraine to assess the impact of the war on stroke care, to fund stroke awareness campaign for the general public designed for Ukraine taking into account current situation; to provide access to educational programs for stroke neurologists and physicians within the country. We are very thankful to Professor Charlotte Sandset  (Oslo, Norway); Prof. Yuriy Flomin (Kiev, Ukraine) Dr. Pavlo Lebedynets ( Kiev, Ukraine),  Dr. Dmytro Lebedynets ( Kiev, Ukraine) for working and succeeding in this.

(ESO and ) TaskForce members for Ukraine believe in science, knowledge, individual and societal health as values to connect people from different cultures and foster mutual understanding.

Stroke science and ESO mission of improved stroke care are the stones in the bridge built by the the task force to protect our values, principles and who share those from violence and inhumanity.

While we continue hoping for an instant, fair and peaceful resolution of this conflict.

The TC Committee prepared the new workshop „Quality management in telestroke” which is released on the ESO website.

In 1999, Levine and Gorman introduced the term ‘telestroke’ as the use of telemedicine to provide neurological consultation for stroke in hospitals lacking this level of expertise (1). Due to a wide difference in stroke care between European countries and the differences in the reimbursement considering the country GDP and insurance coverage, the Telestroke cervices unevenly distributed across Europe. Consequently, the European Stroke Organisation (ESO) Telestroke Committee, following publication of their recommendations report in 2018, surveyed European Stroke Organisation members to map areas of telemedicine activity, the nature of activity, identify underserved regions and provide network examples for those wishing to evolve a new service. The results of the ESO TC Survey will be presented on workshop.

The workshop is specified on the management of the quality of the provided services in the acute stroke departments. Listeners will be able to learn more about the basics and definitions of quality management as well as on telestroke.

Workshop Faculties are Dr. Vaso Zisimopoulou, Prof. Dr. René Handschu and Dr. Dan Ryan.

Vaso Zisimopoulou is the Director of Neurology Department and Head of Stroke Unit for Athens Euroclinic in Athens, Greece. She completed her medical studies in Greece and post registrar education in Neurocritical Care in MI, USA. The realisation that Greece has a diversified coast line with 227 inhabited islands and, as a result, unique challenges in providing stroke care was the primary impetus for her decision to participate in the Telestroke Committee. She is an enthusiast and a zealot of technology in medicine, actively engaging into ongoing collaborations to promote telestroke in Greece.

René Handschu is the Professor of Neurology at the University of Erlangen-Nürnberg, with special focus on cerebrovascular medicine, telemedicine and quality management.  Rene is also a member of the Committee for Digital Neurology and telemedicine of the German Neurological Society and of the certification board at the German Stroke Society. In the current time Dr. Handschu is chairman of neurology department at Klinikum Neumarkt a 500 bed teaching hospital that is part of the STENO-network in Bavaria.

Dan Ryan is a stroke physician and Director of Stoke Services at Tallaght University Hospital, Dublin; a 500-bed teaching hospital affiliated with Trinity College Dublin. Dan has completed a PhD in stroke, specifically hypotensive stroke and lacunar stroke, and also carried out his stroke fellowship in King’s College Hospital, London. He is the National Telestroke Lead in Ireland. His research interests include hypotensive and lacunar stroke and tele-education.

On 24 February 2022, Russia invaded Ukraine.

Since then, many people died, were wounded, and had to flee their country. Also, many medical professionals braved all dangers and the most adverse circumstances to ensure the well-being of the patients entrusted to them and to save lives.

We admire the perseverance, courage, and determination of our colleagues in Ukraine.

The European Stroke Organisation continues to deplore the war in Ukraine and stands by its position that the Russian invasion in Ukraine is unjustified and wrong.

As an academic society, we work together to reduce the burden of stroke and hope for peace in Europe. Our thoughts are with the Ukrainian people and our colleagues, as well as with all Russian colleagues and other citizens who oppose war and condemn this act of violence.

Armed conflict directly endangers patients and health professionals and disrupts the safe and effective delivery of treatment to patients with stroke. We appeal for a resolution of this conflict to stop the enormous suffering.

ESO has undertaken multiple measures to support colleagues in Ukraine and to aid humanitarian efforts:

Our Task Force 4 Ukraine (TF4UKR) is in close contact with colleagues within Ukraine to actively support all necessary relief efforts with a focus on stroke care. The Task Force works closely with our professional group to directly assist medical professionals and patients in need. ESO initiated a dedicated department visiting grant and assists colleagues through a dedicated email contact: ukrainesupport@eso-stroke.org.

As an additional support, ESO will again waive registration fees for any Ukrainian stroke physician (member and non-member) wishing to attend the upcoming European Stroke Organisation Conference (ESOC).

In addition, any member of the Ukrainian Anti-Stroke Association or Ukrainian Stroke Medicine Society is automatically renewed for full ESO Membership for 2023 at no charge. This guarantees first-hand information about ESO and scientific news and includes access to the ESO Stroke Education Platform (eSTEP).

Our free webinars, dedicated to our Ukrainian colleagues and translated into Ukrainian, help to disseminate state of the art information about treatments and patient care.

As a scientific society, ESO still believes in uniting people through stroke science and its mission of improved stroke care. ESO will not exclude individual members who share our values and principles from its educational offerings and scientific events.

In order to further support the Stroke Action Plan Europe (SAP-E), ESO has issued an open call for Chief Operations Officer.

SAP-E is a pan-European project that has set targets for the implementation of evidence-based preventive actions and stroke services until 2030. It was developed by ESO and its partner the Stroke Alliance for Europe (SAFE), as an advocacy tool to reduce the burden of stroke, through the improvement of care at the national and European levels.

ESO is looking for a motivated early mid-career, academic stroke care professional who is interested in actively shaping the future of how stroke is perceived and treated within Europe.

The SAP-E COO will join the SAP-E Steering Committee and work closely with the SAP-E leadership team. The person occupying the position will support SAP-E Co-Chairs in preparing meetings and presentations from a content and scientific standpoint, as well as in communication with National
Coordinators. The Chairs and the COO will work closely with the SAP-E Head Office to align all administrative support and communication.

Open letter looking for ESO bloggers

Since 2016 the European Stroke Organisation has used the ESO Blog as a platform to share news with the European stroke community. It informs the international community about recent publications in the European Stroke Journal and blogs about scientific developments and clinical topics. Besides, the Blog provides coverage about the European Stroke Organisation Conference, informs about ESO workshops, D2D visits, and many more activities . We strongly value the contribution from our ESO members and young stroke physicians and researchers in particular. We believe that their input and social media outreach enforces their position within the international stroke community and contributes to a next generation in European stroke care that is strongly connected.

This fall we are inviting young stroke scientist and physicians who are interested in writing about stroke related topics to contribute to the ESO Blog. The selected candidates will have the opportunity to write blog posts on topics that might be interesting to our readers during the following year and we will recognize their contribution.

As an academic society, we unite to reduce the burden of stroke and today we unite to hope for peace in Europe and to help reducing the suffering. Our thoughts are with the Ukrainian people and our colleagues but also with all Russian colleagues and other citizens who are opposed to the war and condemn this act of violence.

Armed conflict directly endangers patients and health professionals, and disrupts the safe and effective delivery of treatment to patients with stroke. ESO has undertaken multiple measures to support the people of Ukraine and to aid humanitarian efforts.

Aside from donating EUR 10,000 to Médecins Sans Frontières, ESO has now established a Task Force (TF4UKR), which is in daily contact with colleagues within Ukraine to actively support any relief efforts needed with a focus on stroke care. The Task Force will work closely with our professional community to directly assist medical professionals and patients in need. Specifically, ESO will assist colleagues to get in touch with neurological departments nearby and obtain an “observership” status within the hospital. Those who are willing to support and/or sponsor initiatives for stroke patients and stroke physicians in Ukraine please contact ESO at: ukrainesupport@eso-stroke.org.

It is with enormous sadness and concern that we at the European Stroke Organisation (ESO) have witnessed the war in Ukraine. While ESO remains an apolitical organisation focussed on healthcare and stroke science, we deplore this war and see no place for violence in settling disputes of any kind. We are clear in our position that the invasion of Ukraine by Russia and indiscriminate attacks by Russian military forces endangering Ukrainian non-combatant civilians are unjustified and wrong, and we support our colleagues and citizens within Ukraine at this time.

Armed conflict directly endangers patients and health professionals, and disrupts the safe and effective delivery of treatment to patients with stroke. We appeal for an instant and peaceful resolution to stop the enormous suffering that is already ongoing.

Communication remains key to any peaceful solution. We ask our politicians, academic partners, and members to continue working towards a negotiated peace. As an academic society, we unite to reduce the burden of stroke. Today we are united with national stroke organisations in our support for colleagues and citizens in Ukraine and peace in Europe.

ESO actively supports organisations that are setting up relief efforts to help civilian casualties and refugees in the region. Therefore, the ESO Executive Committee has taken the decision to donate EUR 10,000 to Médecins Sans Frontières, which is mobilising to support health needs for Ukraine. We encourage our members to donate as individuals to MSF, the International Red Cross, and other healthcare agencies assisting in the response in Ukraine.

We also plan to activate our professional community to directly assist our colleagues that need direct support, and patients who may need transfer to hospitals outside Ukraine in order to receive appropriate care. ESO will establish a task force to connect Ukrainian stroke physicians and patients with ESO members throughout Europe.

We remain committed in our support for Ukraine and hopeful for a peaceful conclusion to this crisis.
